The dwarf/green seer could be an agent but she also says she hasn't been kissed in thousands of years. But she could be his eyes in the south. High Heart is where a circle of heart trees were cut down.

I have always taken the shadow looming over them as a presence that will affect their lives dramatically, but not necessarily kill them. The Hound does shadow Sansa's thoughts, and the the MTN shadows The Hound's thoughts.

Sansa thinks about the Hound a lottttt in ASoS and AFFC; practically every chapter, and a lot of it is latent sexual feelings IMO. I don't think she views him in a negative light at all and I actually think she's slowly starting to process/realize that she's in love with him.
I always thought the vision was referring to Sandor, Jaime, and Robert Strong. I don't know how Jaime will get involved but perhaps both he and Sandor will defeat Robert Strong, once and for all? I could see the Faith Militant selecting Lancel as their trial by combat champion, and in typical LancelgonnaLancel fashion, will get his arse handed to him and killed by Robert Strong. Assuming Sandor=Gravedigger, I still think he'd come out of retirement to kill his brother/the evil that is Robert Strong. It's also possible that RS will be unhelmed and revealed as a monster created by Qyburn/sorcery and the trial will be invalidated. Either way, I think Cersei's going to be completely stripped of power in TWoW if we are to believe when GRRM stated that multiple butts will sit on the IT before the series' end. Perhaps Cersei will try fleeing to Casterly Rock, but it's a big question mark until TWoW is actually released.
I honestly don't see Cersei surviving past TWoW. Her storyline is near its end/conclusion, and even if Jaime is her Valonqar, I think he'll survive TWoW and die in ADoS.
