I flew Turkish within Europe last year. We arrived into IST from Rome, you will need a visa to enter the country (can be purchased from kiosks in the arrivals hall). I do not think that they have a sterile international hall for transfers, so you will need to clear immigration to make your connection.
Turkish Airlines is a top notch carrier. The service will be fantastic and their 777s are configured with 9 abreast as opposed to a very cramped 10 abreast. Would highly recommend it if only using IST as a layover.
