Excessive iPhone Data Usage

My wife has an iPhone 6S Plus that seems to be consistently using data from AT&T. This causes us to always be right at the limit or over. We have wifi at the house and it's pretty consistent. Like any female, she uses social media ALL the time. She's a teacher, so she's not using a ton of data at work.
Two things that have recently concerned me/blew me away regarding her iPhone.
First, I was updating her phone last month while connected to a computer that was on a wired internet connection. This was on the last day of the billing cycle, so we were right at the limit. I connected her phone, started the back up, and downloading the latest iOS software. I walked away from the computer, and got a text from AT&T saying I exceeded the limit and an additional 1 GB was being added. About 20 minutes later, I got a second text saying that was exceeded, and another 1 GB was being added. Considering we were both home, on wifi, I knew something was wrong. Walked back to her phone at the computer and for some reason, it was not connected to wifi, and the computer was using the phone's data connection as hot spot to sync, backup, and download the latest iOS. WTF.
Today, I was looking at her data usage charts on AT&T while evaluating the new Unlimited plan (much more expensive). I noticed her phone is showing data usage at times she is definitely connected to wifi. For example, between 3 AM and 6 AM this morning (log updates every 3 hours), while we were both sleeping, her phone shows a data usage of 275 Mb. Yesterday, same time, 101 Mb. Day before, 144 Mb.
WTF is going on and how can I stop this? She consistently uses 2-3x more data than I do, and I'm pretty heavy using my phone at work/off wifi. Most of her usage should be on wifi.

Applications is the biggest data sucker. I had this with Verizon but I will admit my apps I used was the reason.
*Reset wireless router at home
*Tell wife to reduce when not on WiFi
*Call them and tell them what's happening. They'll bump you up to next package which will be cheaper, but will only reduce current month.
*If she has a 6s, I believe there was a settings hack to help with this, but can't remember

There is actually a setting that uses mobile data while on WiFi.
Turn that off

My sister had something similar happen. AT&T ended up swapping out her phone for a new one and crediting all the data back to the account.

You have to turn off Wifi assist. When your wifi signal isn't great, it starts using data to compensate. It's a dumb feature.
Turn it off:
Settings--> Cellular--> Scroll down and turn "Wi-Fi assist" off
Also, underneath the wi fi assist feature, turn off the iCloud Drive as well.

Wifi assist is turned off. It's been off for as long as I recall. I've also adjusted her settings in FB to only Auto-Play videos on wifi. And to not use data to download App Store updates. Also "Background App Refresh" is turned off.
I've reset her statistics so I'll check in the morning if any apps have used data since we'll be on wifi the whole time.
It's just frustrating. I'll give AT&T a call as well, but I don't expect any help.

Turn off WIFI assist.
And
This was a few years ago but when I used to have a small data amount I would go over my limit in the middle of the night. I'm pretty sure when your phone is in sleep mode it's not connected to wifi... so any push notifications you get while the phone is in sleep mode use data.

Solution. Switch to T-Mobile. Pay $40 per month per line (I have 4, 2 teens). Never worry about stupid fing overages ever again.

We used to be with AT&T and pretty much the same thing you described about the data overages and such happened to us with regularity. 5 lines, 3 are smartphones. $240 a month. We switched to Cricket. They use AT&T towers so it's the exact same coverage we had. They sent us the SIM cards either free or 5 bucks a phone, I don't recall. $120.00 per month and never go over data limits like with ATT

How much would 4 lines (all smartphones) be? And how many GB's do you get each month?

Sorry so late with the reply. You get a discount for each phone. Ours is 40 for the first phone, 30 for the second, 20 for the third and 10 for the fourth. You can add a fifth line for free. That's all with 3 gigs each. We pay 20 extra because one of our lines is unlimited data and one is 8 gigs. You can change the data plan for each line at anytime during the month. So if you're about to go over, you can up your plan.
Also, don't go to Cricket to get them to activate your phones if you switch. They'll charge you 25 bucks apiece. You can get the BYOD SiM kit on Amazon right now for 2 dollars and Best Buy as well. You would need to make sure your phones are compatible to ATT though. The Verizon phones aren't.
