In simple terms:

On a free kick, you are required to give the kicker 10 yards. He first has to request it, so that is why you will see defenders step in front of the ball. In some leagues, including MLS, they use a white spray to indicate the ten yards.

On that play the linesman backed one player off but not the other two. Then he went crazy and sprayed 10 yards all the way around.

If the kicker doesn't request the 10 yards, he can kick it at any time. When you do request the 10 yards you have to wait for the refs whistle.

Every now and then you will see a fk that the defensive team backed up and the kicker did not request it, so he takes the fk while the wall and probably the keeper is not ready. This happened in one of the USMNT games recently.
