Lobbists/think tanks write the bulk of them, send them to Congressional aides who mark them up. Sometimes congressmen are involved in the mark up, sometimes they're not.

Almost every bill is written by lobbyist or "industry experts" (like there is a difference between the two).

So that's how they keep crony capitalism going, right?
Like this entire health insurance debate. Prior to Trump the GOP congress had 50+ Obamacare repeal bills. Now that they have the White House they can't get one fricking bill passed. Probably because the insurance companies writing the bill don't want to reduce their revenue stream. They rely on government regulation for profits.
