Unfortunately Scalise back in ICU. Condition "serious".

When I first read the title and given this point in time in the recovery process the first thing I though of is infection. And sure enough:

When bullets hit bone, a lot of bad shite happens. Hoping for a full recovery for one of the good guys.

Right.

The bullet hit his pelvis and penetrated the pelvic floor. There are a lot of organs in this area of the body.

Bad things happen to people the longer they're in the hospital.

That's one of the reasons we usually try to get them out asap.

Really unfortunate if they end up losing him to a nosocomial infection after heroics in surgery.

True.
But this is far more likely an anastomotic leak or similar etiology rather than standard nosocomial HAI.

Who knows. I'd wager he's on a TPN at this point due to the nature of his injury and it's not "if" but "when" you get an infection with a TPN.

But it could just as easily be an infection from his injury, too.

If hes in the ICU for an infection, isnt it likely that hes on stuff like levophed, vasopressin or phenylephrine due to sepsis? If thats the case, not looking pretty.

I don't know the extent of his injuries other than he was shot in the "hip area". But what makes you think he is on TPN?

Gut shot, tears intestines up, surgically repair intestines and they need time to heal

Gut wound/surgery makes me question if he has functional bowels. It's plausible there was enough damage to necessitate him being npo for >7days which is the indication.

Not necessarily. Could be any number of reasons. Nonetheless, it's a set back.

Normally in the pelvis, that would involve a colostomy rather than TPN.
