With all of the safeguards in place as described here by Navy personnel, it has to be at least considered a possibility.

U.S. Navy, citing poor seamanship, removes commanders of warship in deadly crash

First, Admiral Richardson said, he ordered an “operational pause” for Navy fleet commanders to review teamwork, safety, seamanship and other “fundamentals” aboard all 277 Navy vessels. Commanders will space out the review to avoid hampering operations, like the war games in South Korea that started on Monday.
Second, the admiral said, he ordered a broader, monthslong review to examine the specific problems with the Navy’s Seventh Fleet, based in Japan, as the Navy has suffered four major ship accidents in the western Pacific since February.
“That gives great cause for concern that there’s something out there that we’re not getting at,” Admiral Richardson told reporters at the Pentagon during a brief news conference.

The Navy is quick to drop the hammer.

As it should be; the Fed needs to adopt this

Is there any way that they could be the result of cyber attacks, i.e. hostile actors gaining control of the Navy ship's navigation system or that of the foreign vessel?
I would think it far more likely that these were deliberate attacks by the "merchant" vessels. Instead of cars driving into crowds, you have ships ramming ships who are not allowed to fire on them under the RUE and Navy is keeping it under wraps to downplay the threat.
However, I think it's far more likely than that that we just have some tards at the helm.

I've been out of the Navy for 17 years, but don't the ships still have INS and Gyro for navigating?
If not the certinanly still have them for backup. If I recall INS only needed GPS for initial calibration, and the gyro didn't need it at all.

My question would be why the Navy dropped the hammer on officers and crew if they knew there was a cyber intrusion and the crew wasn't at fault?

I highly doubt it was hacking of the Navy ship. Prior to my move to cybersecurity, I dealt mostly with satcom and I'm very familiar with the Navy's systems they use to pass data. I just don't see it happening even by a top APT.
Now a hack of a merchant vessel? Sure.

I posted their bios because many years of sacrifice went into their careers, their wives and families as well. Even tho it's SOP for the CO and XO to take blame, I am wondering if their careers will ever be reinstated if foul play is discovered with them and the McCain. Or... if someone was really asleep on watch. There has to be many inept and asleep for such a thing to happen. It makes no sense.

This has to be sabotage. From within: a) possibly a terrorist/enemy infiltrated the US Military? (SEE the Fort Hoood shooter) Or, b) somehow a GPS hack from the outside. Like someone said properly in another thread: they will never admit it in public; but heads will roll and be fired as SOP-- and that is a shame to ruin long military careers over this...
But, this doesn't explain a lack of eyes and proper response by the crews of these vessels.
By all indications, these vessels were detected on radar, but were allowed to get too close with no reaction.
While "tricking" the machines would explain inaction by the crewmembers at the displays, it does not explain why the watchmen didn't alert the bridge.

In the case of the Fitzgerald, it was reported that the Phillipine freighter flashed lights to the USS ship as typical warning beforehand and the signals went ignored and they crashed. It claimed the freighter had right of way. If the freighter saw them why didn't they attempt to change course? What is the lag time of a commercial freighter changing course?
