WASHINGTON — Enrollment in Medicaid is surging as a result of the Affordable Care Act, but the Obama administration and state officials have done little to ensure that new beneficiaries have access to doctors after they get their Medicaid cards, federal investigators say in a new report.

The report, to be issued this week by the inspector general at the Department of Health and Human Services, says state standards for access to care vary widely and are rarely enforced. As a result, it says, Medicaid patients often find that they must wait for months or travel long distances to see a doctor.

The inspector general, Daniel R. Levinson, said federal and state officials must do more to protect beneficiaries’ access to care, in view of the program’s rapid growth. Just since October, the administration says, eight million people with low incomes have enrolled. By 2016, the Congressional Budget Office estimates, one in four Americans will be on Medicaid at some time during the year.

The federal government “uses a checklist to confirm that states have access standards,” but does not analyze whether the standards ensure access to care, Mr. Levinson said. Federal officials, he said, view the setting of standards as “primarily a state responsibility.”

In assessing whether health plans have enough doctors, states typically rely on information supplied by insurers, Mr. Levinson said, but “this information is often inaccurate or out of date.” Some doctors are no longer in health plan networks. Some are not taking new Medicaid patients. Some were never at the address listed in the insurer’s directory of providers.

This is a rationing program instituted by the suppliers instead of the government. Thus, it is bad rationing because the bureaucrats do not control it. The bureaucrats will want to mandate that healthcare providers must take Medicaid patients so the bureaucrats will have control over who gets treated, what they get treated for and how much money the treatment will cost. That has always been their desire.

The interesting conflict will be how they balance the rationing of care among Medicaid and Medicare beneficiaries. Senior citizens get priority over the poor currently, but will that always be the case? Arguments are already being made about the costs versus benefits of extending the lives of the elderly through expensive treatments.

As a physician why would I see a patient for 50% of what I get from other insurances? Not financially smart.

Exactly...

Until the Govt mandates that in order to practice medicine, you have to accept medicaid.

better get used to it, because as long as one side equates having an insurance card with healthcare then the quality of care is only going to go down.

Damn right it is. They pay providers shite. Here BCBS gives me about $89 for a 99213 office visit. That's the most common. Medicaid? $37 and they tend to be less desirable, less compliant, sicker pts. Medicare pays $67. Oh and they only get 12 visits per year and 3 meds per month unless you fill out a form to get them 6, but they have to be six long term meds. And they will fricking deny everything they can AFTER the care is given. BUT....they pay specialist damn good for procedures and they are the best paying insurance for pharmacies.

Pay us a decent amount and we'll take some. Or I can see one easy, "good" BCBS pt for what I can get for 3 medicaids and have a much wider array of treatment options. Medicaid is shite insurance.

This has been said by those against Obamacare from the start. You have many doctors that have retired, you added millions of new patients and doctors are being paid less to treat them. It takes many years to train and add new doctors. Doctors have options, squeeze them and they will leave the health industry entirely...

Boo fricking hoo.

Doctors don't take a vow of poverty. Reimbursement rates are already shite...and that doesn't even take into consideration the paperwork nightmare.

The government is really out of control the last few years when it comes to adversarial paperwork.

Medicaid is an essential weapon in the War on Poverty. Unfortunately it is not a very good weapon since it does not kill the poor fast enough.

Medicaid pays like 10 cents on the dollar while Medicare pays maybe 35 cents and real insurance pays maybe 40-50

Not a shocker doctors try to avoid medicaid at all costs
