Daughter is struggling academically in school - What do we do?

I was hoping someone that has been through this could help.
My daughter, who is in the fourth grade, is struggling academically. She has always struggled in school but we thought it was because of the school itself and moved her to a better one this year. She got her first progress report in today and she is failing big time, averaging 40-50 points. She doesn't seem to be able to grasp basic concepts in math and reading.
What should we do? Should we put her in some sort of after school tutoring program? Should we get her evaluated for learning disabilities? One thing we will do is now is start working with her one on one ourselves so hopefully that will help.
I'm at a loss. I actually feel like I'm a horrible parent for failing my child.

I would try an after school tutoring program. It's more popular than you think. She will be analyzed over there on her learning abilities and you can go from there as far as her progression goes.

outside help and maybe have her tested for this and that. try to help yourself but you probably be better getting outside tutoring

have you considered talking to the administration and her teachers and possibly sending her back a year?
