Why the hell they need to know my wife's place of employment, address, ph.#, date of birth, social, etc.?
I'm getting my freakin' shoulder looked at.
When to a dentist awhile back and one of the questions was, "Do you use alcohol or cocaine?" This was a yes or no question.
WTF?

Because if you need certains meds it could greatly frick you up and kill you. They need to know not to give you certain meds if you're on these things.

You really think the Doctors or anyone in the medical field came up with the paperwork? You have to fill all that crap out because almost always its the law and/or it's required by insurance companies. I didn't get into the medical field to do paperwork or read all of your paperwork for that matter.
