How does this even happen? Coupler not latched? No safety chain? Why the tow?

I remember going on a fishing trip with my neighbor when I was about 12.
Crazy bastard on the ramp next to him at launch was putting boat back on trailer.
He gave it way too much gas, and launched the boat right up and off the trailer.
The front of the boat flew over and smashed out the side window on my neighbor's 4-Runner.
That window costs just over $700 to replace!
