What is the most likely cause? Provider, Router (very old), or Modem, or other settings related issue? Thanks for any advice.

If you're losing wireless connection, it's the router, could be bad or could be interference. Try setting the wireless to a different channel(1, 6, or 11). If you're losing internet connectivity, it could be any of those 3.

Well, "Router (very old)" doesn't sound too good, but just for the hell of it, let's get some more info.
1. Make and model of the router?
2. Does this happen with more than one device, or are some devices working totally fine with the router and others aren't?
3. What devices are experiencing the problem? (particularly, model numbers of any laptops, and the OS they're running). Also, name the devices that seem to work fine with the router, if any.
4. Internet provider company? Modem brand?
5. What do you mean by "losing Wifi"? Do you see the signal on your device(s) disappear, or does the internet stop working even though you still have an active, strong connection to the router? Error messages? How often does it happen? Just need more details on the symptoms.
6. Have you tested hard-wired (ethernet) connections thoroughly enough to confirm that they are 100% reliable (or at least far more reliable than wifi)? i.e., Have you tested ethernet directly to the modem as well as to the router, and are there performance/reliability differences?
7. Where is the router located? How far away is it from where you typically use your device(s), and has distance made any difference?
