Letter to end-user re materials sold to contractor?

I once saw a letter that a material supplier woulf mail to the homeowner, letting the homeowner know that their contractor had purchased materials and they had not been paid for yet.
Does anyone know what this letter is called?
Id like to duplicate one for my own use.

Letter of demand for payment. At the end of the day the supplier may place a lien on the home and the homeowner will need to sue the contractor.
