I had some friends going through med school that worked with people around that weight. They told me that the best thing anyone could do for them was blow their head off, because it's abject misery.

I saw one where the husband didn't want her to lose weight and would constantly try to tempt her to eat bad foods after her surgery.

He was a VERY dedicated chubby chaser

Even so, that's sadistic.
