I think it was mw2, the online multiplayer system was down the first 2 or 3 days after release. That sucked.

World of Warcraft had a way worse launch than Diablo 3 did.
Blizzard handed out so much free gametime to players on certain realms. It was BAD.

Duke Nukem Forever, mainly for the 100 year delay.
Diablo 3 was delayed over and over and they finally just pulled out half the fracking features and released it.
Latest Sim City is up there near the top of worst releases ever.
