Before I start ordering, do I have to replace the entire hub assembly to fix a bad front wheel bearing? That appears to be the case, but I don't want to spend an extra $200 if I don't need to.

No, you can drive out the bearing cups and install new ones with the bearings.

From past experience with a four wheeler, buy the new hub.

Buy a new hub or go to salvage yard and pick one up that has at least a 90 day warranty. Can buy three at salvage yard for what a new one costs.

If you want to keep the truck for a while longer, yes.
Go with a OE Ford one, stay away from the cheap, made in China crap you get at Walk-Home Auto Zone.
