Having a homemade boat inspected by LDWF

anyone have experience with the process?
Do i just fill out the affidavit for boat inspection,have it notarized and mail it and wait for someone to contact me?

I think you use this form LINK, then they will send a game warden out to put a sticker on the back and stamp a HIN on it. It's a pretty easy process.
ETA: I had a receipt showing the aluminum that my boat was built from.

That's the form I meant to link, not sure why it linked the other one. I have the material receipt ready to go. Did you have that form notarized?

Did That and an agent came out to the house and looked it over.
