Can someone walk me through how this happens? I've only ever launched lighter boats but how do so many people per year end up sinking their truck?

Three weeks ago I ready my boat for launch in the parking lot (not on the ramp because I am not a chode.)
I back down the ramp and with my trailer tires in the water I stop, put on the emergency brake, and jump out of the truck to unhook the strap holding the boat on the winch.
When I get back in the truck I realized I left the truck in reverse. Thank God the emergency brake held.
My point is it is possible to sink your truck.

Is it only caused by leaving the truck in reverse and not hitting e-brake?

This has to be it.
I'm not saying I'm doing it right, and I may be trusting my parking brake too much, but I've been doing this for a long time. I almost never use my parking brake. I always place the vehicle in park first, even if I do use the emergency brake. Never had an issue.

This is the part I'm not getting. I have never once in my life gotten out of a vehicle without putting it in park. It's just reflex at this point.

We have all done it. Sunday my buddy backed the trailer down the ramp and as I was driving the boat towards the trailer he was standing next to the winch stand with his reverse lights on. It happens, even to people with lots of experience. My dad even puts a chock down anytime his truck is on the ramp.

I did do it on my SxS up in the mountains. damn thing almost rolled back off a cliff
