This might only be part of the whole story, but a few weeks ago, the quarterly earnings came out for the airlines. While Delta and American came out big, United posted a huge loss (think it was over $500 million). Since then, United's tried to raise prices obnoxiously high to make up for that loss. Meanwhile, AA and Delta have increased their prices to keep on par with United, but not nearly as high. This is only the case on certain routes, you can still find good deals to various places.
I could be completely wrong, but that's what I think is going on.

Because people are fricking fat these days, and fat people on average require more fuel to move their fat asses through the air.

$400 is actually not bad considering you are trying 2 weeks out
My flight that I booked back in March from Phx to BR was $360 total.

I just looked for shits and giggle and it's $299 or 18,232 pts one way out of Houston Hobby and basically the same thing coming back. Not bad at all.

Plus reduced capacity. I have taken 65 flights this year and only one or two have had more than 10 empty seats. The folks in revenue management at the airlines are doing work!

Airlines have virtual monopolies at many of their hub airports. With very little competition at any particular airport, they can charge high prices. And during the recession, most airlines reduced the number of flights per day to any particular destination and hence the number of available seats. With more people flying now, they haven't really increased the number of flights so more demand = $$$.

No its not. If you book about 45 days out, you can get much cheaper than that. I'm pricing a flight to Milan in the beginning of July and I've found flights for $1075. I flew into Charles De Gaulle in February and it cost about $1100.
