At LSU I had a professor who was from Chicago and told her regular (within the small group of majors in the field) to call her by her first name.
I could not do it. To me, she was then and still is now (17 years later as a Facebook friend,) "Miss Trish."
A Yankee friend in the group joked once to the others, "its 1999 is good people. You know how I know? She says 'Miss Trish'."
Mr or Miss first name if you know them and like them. Last name if you don't.
Hell I call my kids Mr and Miss half the time.

I read your post and now I can't for the life of me recall what I called my college professors. It's weird. I think I barely addressed them at all now that I think about it.
I know I did call Barbara Apostalu "babs" upon request.

I was raised to address adults properly. Now that I am an adult I no longer use the Mr/Mrs (first name) unless I'm joking around because I'm an adult and if we are on first name basis then it's not needed. To me it is very unprofessional in the business environment to use the Mr (first name), if you are going with mister then use the last name.

Never was required to say sir/mans when growing up in the south; Yankee parents.
However, I always have and still do use it where appropriate particularly when addressing women.

M/m first name if freinds and I still address everyone that way.
