Usually how we were introduced. Usually M/M First.
Only those you didn't "know" were M/M Last.

In Louisiana, Mr.First-Name/Miss First-Name (not the drawn out Mrs., even if they were married).
Around my Yankee relatives, Mr./Mrs. Last-Name. I don't think I ever would've heard the end of it if my grandmother heard me using an adult's first name, even with Mr./Mrs. attached.

Always Mr./Mrs. First Name. I think it's the formality of calling someone by their last name is weird when you're friends with them.
ETA: When I'm an adult I'd want my friends to call me Mr. First Name. Whatever name you call someone has nothing to do with if you respect them or not

When I hear a kid not give respect to elders I want to disrespect them.
Badly. In front of everyone.
Yes sir.
No mam.

Where I'm from there's like four or so prominent last names so calling them Mr. (Last Name) would just be weird. It was always Mr. (First Name).

I've always done the first name. In Alabama, it's the last. My wife and I were dating at the same time her sister and now husband were dating. I always found it weird that we didn't get on the same page with that one, but I wasn't switching. Now we both just call them by their first names only, no Mr./Mrs.

Mr. and then first name. Unless other people used their last name to address them. But yes sir and no sir was a requirement of my up bringing.

I grew up in mid west. We called adults by their first names.
