Seems like such a difficult thing with clubs. Why not just toss the ball with your hands?

precisely. there should be a weight shift at the beginning of the downswing. your upper body should feel like it does absolutely nothing. but what happens in reality is you get a lateral side bend which effectively drops the club into the "slot." at this point you proceed to try to knock the shite out of the ball while keeping your head steady and not moving forward. essentially, an inclined baseball swing.

I've never had success with a weight shift or hip rotation as a key.

It always forces me over the top.

Your lower body is the engine for a solid, repeatable swing with "effortless" power, there's no way around that. It could be ball position or you swaying off the ball in the backswing. Try moving the ball forward of center for all your clubs to make it easier for your head to stay back. Difficult to know what to tell you without seeing your swing.

I know that everyone has their own things that work for them, but something that helps me is "aiming" a couple of inches in front of the ball.

Weight shift is the key. Keep your weight back on your right foot as the club slides through the ball. Simple....
