That is the last for a bespoke shoe - you can see that one has the detail for the brogueing drawn on the toe. Now maybe there are some bespoke shoemakers that give a customer the last to use as a shoe tree, but it would be fairly impractical, as you can see there is not really a mechanism for removing the tree easily from the shoe, not to mention, the next time the customer wanted to commission another pair of shoes, the shoemaker would have to go through the time and expense of making another last (form). Typically the company will keep those at the shop just as a bespoke tailor keeps the form for a suit.
And you are not getting bespoke shoes for $1000 - think more like $4000.
