PTs or Ortho Docs, immense shoulder pain. Help!

First off I had surgery 2 years(may 1st, 2013) to repair a full labral tear, partially torn rotator cuff, tighten the subscapularis I think, and multiple bone spurs. Something went haywire. It's been hurting ever since and I have a 30 lb weight restriction on it.
Was playing basketball tonight, and one of the guys I was playing with hit me in the middle of the collarbone with his elbow. After a few minutes it was like something stabbed me dead on the labrum. I couldn't lift my arm halfway to the shoulder, or even move it away from my body without wanting to scream. It actually wouldnt go over my shoulder, I couldn't make it. Just wouldn't. The pain started in the shoulder and went all the way down to where the bicep connects at the elbow. I laid on the ground and waited for moments when the pain was less and slowly moved it up over my head, using the ground to hold the weight. I can put a finger dead on the spot it started. It was slightly forward of dead center of the shoulder right where the curve starts as it becomes your arm.
This is excruciating. Worst pain I have felt in the shoulder since I first hurt it, it's not even close actually. This made me whimper like a little girl.
After the pain subsided I was able to finish the game, but I could only shoot. I was careful with it shot from lower than I normally do with my jumpshot. Didn't rebound or try to block any shots with that arm.
Still can't move it 100%right, it's closer to 70% of my normal, and I can feel the twinge still in there. It's ready to freak out on me again. I don't want to start all of this again.
Is it shoulder Aids?

And I don't like walking away. I can be hyper competitive at times. Plus I am finally starting to get good again, so I like playing while I can. I've lost 43 lbs in 2.5 months, my speed and endurance are coming back. So I kept pushing...

Sounds like you may have torn it again. Wife's ortho said (one of the best shoulder specialists in the country) said he usually has to repair labrum tears every 7-10 years.

Pain was different this time than the original tear. It felt more like something was caught in that area and wouldn't let go.

Maybe it's not your shoulder this time. The pain sounds exactly like what I had. Even the docs thought it was my shoulder. A PT guy told me it sounded more like a pinched nerve in my neck, even though the pain was at the shoulder and down my arm past the elbow. He was 100% correct. An anti-inflammatory cleared it up w/in a week. I was miserable for over a month, tho. Damn, that hurt.
eta: weird thing was, as long as I was using the arm, working outside or whatever, it didn't hurt much. Let me get still, though, and it was excruciating.

I was wondering if that was a possibility. It might somehow make sense of it being fine before taking the elbow to the collarbone. 4/5 or so minutes later...and boom

Nothing yet. Though, to try and find some bruising, I rotated it around poking and prodding around. I found the spot where it started. When I hit it shivers went down the arm. Similar pain but in more waves this time instead of the sharp line.

Do you have a large step down gap between the end of your collar bone and your shoulder? (if so, thinking AC jt separation).
Could still be neurogenic pain and weakness, similar to, but different than what is known as a brachial plexus injury. Doesn't quite fit the pattern (many patterns)of thoracic outlet syndrome.

Obviously the RICE method for the time being but if it's not swelling by the morning count your blessings.
It sounds like you took an unexpected load bearing hit directly to your shoulder. It could have severely strained those tendons and ligaments as well as a possible partial tear. If it was completely torn I don't think you would have been able to finish the game.
