Oxygen and catheter tubes mixed up resulting in death

Not even possible. Maybe if it involved duct tape and JB Weld but even then it's a stretch.

I saw a joint commission article once with a list of stupid mistakes. One was someone attached the catheter to the OG of a sedated patient. Another one was attaching the IV pump to the ET cuff pilot balloon (that patient died). There were about 15 incidents that were all rediculous but the majority of them happened when family was just trying to help out but didnt know what they were doing.
