OT Runners: help me diagnose a foot ailment

For the past few years, whenever I try to run (or walk a few miles for that matter), I get a sharp pain on the side of my right foot. The pain starts just below my right ankle and will gradually spread along the side of my foot.

Even if I go a few months without running/walking, the pain will come back almost immediately when I start up again.

Any thoughts?

Is it the inside or outside of your foot?

Is it worse barefoot or with shoes? Any swelling?

What type of shoes do you wear?

Outside of right foot.

No swelling, pain subsides pretty quickly after I stop.

I'm wearing shoes, because I'm trying to run or walk for exercise. I can go play tennis for an hour on hardcourts and not feel a thing.

I'm wearing Addidas running shoes right now.

In-between high arches and flat-footed.

Without hashing out a detailed history and no exam....

It sounds like peroneal tendonitis.

The pain is usually between the lateral malleolus (outside ankle bone) and the base of the 5th metatarsal (which is a bony prominence which can be felt on the outside of the foot) where one of the peroneal tendons inserts.

It's more common in people with high arches, because the peroneal tendons prevent your foot from rolling in, which is more common in cavus feet (high arches).

If you do indeed have higher arches I would recommend a more study shoe like a Brooks beast and a decent otc orthotic like a Spenco to keep the tension off the peroneal tendons

Usually means you aren't running long enough and putting enough stress on your ankles to build up strength.

When running, make quick changes of direction and sprint across uneven land if you get the chance.

Asked wife (no pics) who is a podiatrist. Says it could be a number of things, starting with gear/shoes, running surface, or peroneal tendinitis.

In other words, go see a doc and get diagnosed.

Have high arches. Started running barefoot. No problems ever again.

Gotcha .... but what doesn't make sense is that I will not run for 6 months, but then the pain will come back as soon as I try to start up again, without really even pushing it hard.

It has to do with the stress placed on the peroneal tendons (the peroneal brevis in particular) while you are running

They are having to work much harder to prevent inversion of your foot versus simple walking.

That's why you would probably really benefit from a stability type shoe which prevents as much motion in the heel

I have pain in the same area. But it's not a sharp pain and it rarely ever happens during the run. It's afterwards or the next mroning. And it's just a soreness. Nothing overly painful. But I was concerned about a stress fracture since it wasn't going away.

Went to a sports medicine place and was given anti inflammatory medicine. Told it was likely a "stress reaction", or tendinitis. Took the meds and it was gone nearly instantly, so I pretty sure it's just tendinitis. But it keeps coming back.

I ran in Newtons for several months before it started. Switched to Altras and had a few weeks with no issues, but it's back again. I rub a topical anti inflammatory liquid on it to keep it under control easily, but trying to work on my foot strike to make sure I'm not putting to much stress on that part of my foot. Trying to hit more mid foot than forefoot.
