Plenty of conservative physicians or nurses would tell you 2&3 aren't worth it, although obviously you can mitigate risk. Seems like the football evidence is there overall, I doubt a year of middle school football really poses much risk.
But what's the rationale for #4? Because they're scary?

My suspicion is that shooting a shotgun leads to hunting which leads to hunting accidents etc.

The risk isn’t recoil but overpressure from the explosion. Large caliber rifles have been proven to cause TBI over time, I doubt a kid shooting a .22 is any risk but there is probably an argument for a child shooting a standard rifle round.
