I know everyone here is a surgeon on every other weekend.... so help me out

My sons knee got twisted. It also got bruised. X-ray came back good, 1 week after injury the doctor did all of his physical tests... all the twisting and turning of the knee. Says everything felt intact, needs rest.

Within 24 hours of exam, knee pain is worse, considerably. Knee now pops and the ligaments "crackle" when pressure is applied to the knee. Pain opposite of the LCL now (lcl strain suspected).

So is this expected for a strain? A compression sleeve helps a LOT but when it's off the pain goes way up.

X-Rays are worthless. Need a MRI of the knee to show ligaments.

X-rays on a knee are pointless. But all they can do in an ER or urgent care.

He needs an MRI. May have torn meniscus or a floating piece of meniscus. Neither would show in an X-ray.

I'm not a Doctor.Just someone who ruined their Rt Knee playing BBall and got a Knee Replacement last November at age 54.

I was probably a few years older than your Son,but not that many, when I could feel things popping in my Rt. Knee when I bent it back and forth,I could feel it when I put my hand on my knee while flexing it.

I tore the Meniscus and had Arthroscopic at age 20 when it was just coming out.In 81'.

Best of luck.

And like someone said,MRI.

He's gonna need to rest it either way. I would possibly start trying to get approved for an MRI. Not that he will need it stat.

Pain could be coming from meniscus tear or from extra fluid and bone bruising. Pain doesn't actually correlate to a ligament tear, but that's not to say he doesn't have one. Definitely go MRI but also ask your doctor about the possibility of a posterolateral corner tear (not to be confused with the PCL or Posterior Cruciate Ligament). This won't show up on an MRI because it is behind the hamstring. It's actually very rare to have but if torn and not repaired it will only lead to more ligament and meniscus damage.

An mri should be the next approach if there was nothing found from the physical exam.

Except maybe insurance won't pay given negative physical exam. Often times, insurance companies will cover MRI for evaluation after failed conservative treatments or persistence of symptoms.

That's the impression they gave me. Follow up question, how long should compression sleeves be worn at a time? The leg throbs when it's removed but not when on.

From your description above sounds like a medial meniscus tear and an LCL strain.

if rest for 2 weeks does nothing and the edema remains he will need MRI. ICE with frozen peas for now.

compression can remain in place 24/7 as long as there is no vascular compromise. check for appropriate capillary refill in the exposed toes by pinching the finger nail and observing how quickly the blood returns to the nail bed once released. Should be brisk and less than 3 seconds. Also should be same as on contralateral foot.
