Instigating Ejection Question?

I hope this hasnt been discussed- but how often are people ejected/flagged for instigating a fight? I have thought and have heard for years, that the second punch gets the flag- why was this any different? or why wasnt the penalty given to both 51 and Gronk?

I am pretty sure of this...if not a punch...but second shove for sure.

Example: two players get in each other's face, one bumps/shoves the other one- the flag comes out when that person shoves back - very normal occurrence, i have yelled many a time if the second offender was on a team I was cheering for

They flag what they see. 9 times out of 10 they see the retaliation and not the original shove/hit.

They say that because the official typically doesn't see the first offense.
