From your example, repercussions could be very negative and severe.
Walk away.

Grab the aggressors shirt abruptly, pull him in close for a headbutt and bail.

I'm a ray of sunshine no matter what is said until it gets physical.

You can literally be an inch from my face screaming and won't get a reaction.

Younger me used to choose that moment to take a sip of beer or puff of cigarette with my right hand while prepping the left to swing. If the disinterest pissed them off enough to swing, they got punched, beer/cigarette to the face, and usually a kick to the right ankle as hard as I could manage.

AVOID at all cost those types of people who live their lives in turmoil to begin with. Instead, surround yourself with people who want to avoid such altercations and enjoy life instead, who are more easy going than confrontational.

In adddition, avoid those places where such people gather or else the sickness will infect you as well, or at least cause you distress.

A few years back, I went to a game with a friend of mine, and we went to game day at Walk Ons near campus. Lots of drinking going on, and irrational drunken testosterone overflowing. I went to the pisser, and while standing in line trying not to piss myself as I stood in line, I encountered “Anger Man” who came up to me and asked me if I was bowing up to him, to which I said no, just wanting to relieve my bladder. As I walked away I realized that I had put myself in that situation considering the knuckle draggers that had congregated there, and one who thought nothing of wanting to start shite in a public bathroom of a restaurant with families eating right outside the bathroom. Instead of blaming him for being the idiot he was, I eventually took it all upon myself for knowing better than to go there in the first place.

Distance is your best friend with these types. Use it.

FYI, they made a song about this situation

It was created by a very astute artist known as Little Jon.

I believe the name of the song was called "What U Gon' Do?!"

I can completely see this if he was right out of boot. Once you get past E-4, you can generally walk away knowing you could easily rip 99.9% of the human population a new arsehole though.

He was about 6 months from being out.

We have proven a few times, he can beat me in sit ups, rope climbing, and running, nothing else

Definitely not the art of drinking.

Do you risk taking first punch and getting blamed for the fight or wait to let him take first punch?

Honestly I would walk away. If he isnt an immediate threat to my safety I have nothing to defend against.
