If my GF wanted to get an ID card that has my physical address, does she need to provide any additional information?
Can she just go in there and say i need an ID card and just put down the address and bam you are done?

I got an La driver license last week. All I had to do was write my address on the registration form. I don't see why it would be any different.

Not sure how it works in Louisiana, but in DC you have to show proof of residence and address (lease agreement, deed to house, etc.) before they will give you one.

Yep. That's if her current ID is still valid. Otherwise, she's got to go to the Main Branch.

I'd bring a bill with her name on it and your address just so they didn't give me any flak. Probably could just say you moved though.
