While Italy has traditionally not been a super great English speaking country, that has changed, especially in the cities. Most people speak at least a little English.
I would learn basic phrases and also have Google Translate on hand.
Duolingo is a good app.
It is more important to learn TRADITIONS and COURTESIES of a foreign country than the language in many cases, imo.

I would combine Duolingo with something like Rosetta Stone. I've tried Italian with Duolingo alone and the conjugations and tenses killed me. It glosses over that part too much for me. I think it would be good combined with something else.
Also, if you can find TV channels in Italian to watch as you learn it would be beneficial. There's nothing like immersion for faster learning.

Do you know any Spanish? I can send you some good Youtube channels that are for beginners
Do you have any knowledge at all about the language? I can give you some phrases that will help you a lot that aren't really taught in those lessons that I think are super helpful.
Just a little humble brag, but I speak it pretty well and go to Italy quite frequently for work and vacation. Hopefully I'll be able to help out.

I posted a few days ago about suggestions for learning German.
The two most helpful responses I got were:
1) Duolingo. It's fee, and it's actually pretty helpful. I'm almost 1 week through and I seem to be doing fairly well.
2) "Slow News Podcast". One poster mentioned a specific one for German where they read the news, in German, very slowly. The Podcast description is a transcript of what they're reading, so you can follow along, translate, and focus on pronunciation. I'm not there yet, but I can see how that will be very helpful when I get to that point.

This may not help you, but if you can speak Spanish you'll be fine. Spanish and Italian are so close I was able to have functional and at times recreational convos where I spoke Spanish and they spoke Italian.

This backfired on us. We were in this huge line that wrapped around the building and everyone was paying cash at the register. All we had at the time was usd and credit card. I asked the lady at the register with my credit card "necesito euro?" She said si and so we started to walk off. Then she told us card was fine. So we pay then rush up to the counter and then lady asks us which flavor we want in Italian. My buddy continued to trip all over himself and I wasn't helping by just yelling out flavors behind him. Somehow he ended up with 3 scoops of lemon, pistacchio, and vanila creme. To avoid confusion I just asked for the same despite that sounding like a terrible combo. It ended up still being pretty good

Even though I've spoken Italian my entire life, I found that I could have gotten by fairly easily speaking English in Milan, Rome, Florence. Just be advised the farther south you go, the less the people will make an attempt to speak English. The exception would be Sorrento. Napoli, forget it.
Sicily is a crap shoot. Places like Taormina-English will work fine, not so much in Giordani Naxos.
Look, there is a girl from Rome who has a YouTube Channel called "Learning Italian with Lucrezia". I highly recommend watching her videos. Start with her beginner videos. 7-15 minutes. With the little bit you'll learn, you'll be fine.
