Opalescence is good, also for extreme cases see if your dentist uses kor whitening, really good system.

I use Charco Caps, which I get from Walgreens.
I just pop one open and brush away. Looks freaky as hell when you do it, but your teeth will feel great and you'll see immediate results.

Brush daily with a paste made of baking soda and hydrogen peroxide. Do NOT swallow any of it.
