Scruffy's sister wants to get a PhD in organic chemistry. If she pulls that off, he definitely thinks she earned the right to be called Dr.

A doctorate holder is a real doctor, just not a physician. While I think it's a little pretentious, they earned that right.

An MD puts in the work to get the MD.
A PhD in History puts in the work to get a PhD in History.
BOTH earned the right to use Dr.

My son is a doctoral candidate at Johns Hopkins University in the field of neuroscience. He knows professors who have won the Nobel Prize. In a professional setting they address those people as "Doctor". I believe they deserve the title.
I know non-practicing attorneys who have asked to be addressed as "Doctor". I think that goes a little far despite the fact that they have a Juris Doctorate.
