This, for sure if you are looking at USVI. St. John has some very nice houses for rent (have their own pool, option for staffing or not - most won't be on beach, but not a big deal on this island. Caneel Bay there is excellent if you want a resort, and doesn't have to be all-inclusive - it is a Rosewood Property which is a plus.
Take a serious look at Jumby Bay - its a private island resort off of Antigua and is a Rosewood Resorts property. Very low key, no night life, and it is all-inclusive, but the food is actually worth it there.
The Four Seasons on Nevis is also a top-notch trip, excellent if you play tennis and want to break up the beach scene a little.
Parrot Cay Turks and Cacos - another all-inclusive, but so easy to get to, and just an amazing place. Expensive, but another all-inclusive that is well worth it.

If cost is not an option then rent a boat and cruise the islands.

Grand Cayman or Turks and Caicos gets my vote. Hell no to Costa Rica for me.

If you are interested, my dad puts these types of trips together for a living. He guides most of them. My brother is taking a group to Costa Rica in February. My dad is currently hoping around the Carribean right now. He has been to every continent. He wrote a book on backpacking that got published.
I could give you his blog info. He has most of his trips posted on it.
Like I said before, he mostly guides and plans trips abroad. He is all about doing it cheap but his customers seem to love it because most have went back on multiple trips.
Even if you aren't interested in that type of traveling, he does know what he is doing and can probably help you out.

Go to Tortola in the BVI and charter a 45' boat from the Moorings for a week and go Island hopping. Can get a captain and cook.

St. Maarten is gorgeous right now, but it looked like a desert last year because there was the worst drought in the recorded history of the eastern caribbean. Plus if you get board on St. Maarten you can day trip to St. Barths or Anguilla real easy.
ETA: If the OP is scuba certified there are alot of scuba places around St. Maarten and if he is certified advanced he can go scuba diving in the reefs and cliffs of Saba when you come to St. Maarten. Saba

Definitely check out St Maarten.
The full blown nude beaches were something we did. Everyone should experience that at least once in their life.
