A well made/adjusted splint should not hurt your gums when you wear it.

Did you ever discuss this with the dentist?

My dentist did something where he drilled to grind the grooves of my teeth so they wouldn't clench as tightly as they were. It felt like he made my back molars not touch/out of alignment ever so slightly. He had previously recommended a night guard, but I was leery of the cost and if it would truly work since I'm a continual grinder, day and night. I think it helped some, but in hindsight, it doesn't seem to be a very popular way of handling TMJ, as I've never met anyone else who has had this done.

I've moved states since I got it (and stopped using it). The pain is not really prevalent anymore. The main thing that makes it annoying is not being able to open my mouth wider than about an inch and a half. Haven't gone to a new dentist since (which is an entirely separate issue...).

What you are describing is an equilibration and it is a more common treatment than you may reallize. This in conjuction with splint therapy is a conservative treatment that can help most TMJ patients.

I would certainly try this first before having all of my teeth crowned.

Nice. That doesn't look too bad but it looks to open her bite 4mm or so based on that picture.

Looking at the teeth in the image, it doesn't appear that she has had much dental work on them. How does she feel about having all of her teeth prepped for crowns?

Certainly something to consider, since she looks fairly young and dentistry doesn't last forever.

She hates that her natural teeth with be ruined. I think it's an up-sell. It you noticed, only the front six have the orthotic. We are open to alternatives.

Since it was placed there, no more toprimax, and no more Imitrex. She used to pop them like candy.

That's my paradox.

Tough spot definitely.

I'll send you an email in the morning. You may be able to achieve the same end with transitional bonding. Look for my email.

eta: She sounds like a clencher. That nti device does the same thing but is inexpensive and removable.

She might also consider Botox to weaken the elevator muscles and reduce clenching force.
