There were a lot of times when they essentially had decent shots in the mid-range or around the basket but they've been so conditioned to kick the ball back out to three point shooters.

Broken gameplan. They don't shoot well enough from three to take that many of them. When defenses tighten up, or they're just not falling that night, they have nothing to fall back on. You need a plan B in the playoffs, and D'Antoni has never had a plan B.
