Rangers vs Yankees:
Chris Capuano. First inning. 42 pitches. Walked 4. 5 runs. Couldn't finish the inning. Walked in the last run. No reach-around.

ETA: It would appear Martin Perez was not to be outdone. 6 runs in the inning immediately after the Rangers leadoff 5-run inning. 6-5 Yanks, second inning. No outs.

I kept thinking they were going to bring out whoever threw out the first pitch since nobody else was warmed up.

I'm 99% sure I saw a Pirates pitcher give up 9 runs in the first two innings earlier this year
