They do, JR Smith was suspended for failing a drug test with regard to marijuana when he was in New York.

I don't think KD has fallen off or anything but ever since he won the MVP he has completely changed.

Yea, at worst it's very lax. I just read that you have to fail 4 times before any suspension and you are only tested 4 times a year. Not sure what the bench mark levels are.
