Possible Identity Theft- Needs some advice

One of my credit cards got declined so I called to see what caused it. Chase fraud department informed me that someone from Texas called in referencing that credit card number, gave them my SSN, and changed to a Texas address.
I just checked CreditKarma and see that the Equifax report has a Texas address listed (Transunion report lists my current address). Other than this, I have not had any accounts open in my name nor fraudulent charges committed on any of my cards. Someone has my info though, and Equifax has an incorrect address that I assume is the same address used when the person called Chase to change the address on my card.
What should I do? I've never experienced identity theft before, only stolen CC information from time to time.

First, Call one of the credit reporting companies and place an initial fraud alert. They will alert the other two and it will be very difficul for anyone to open up a new account in your name.

I'm going through the same thing right now.
Put a Fraud Alert in with all 3 credit agencies. You only have to file it with one, and they alert the other 2. They'll contact you to verify any new credit applications for 90 days. This also allows you to obtain your full report from all 3 for free.
Call all your credit card companies and alert them and ask for new cards to be issued.
Change all the passwords on your online accounts.
That's all I've done so far, and it appears they've moved on. The attempted fraud occurred 2 weeks ago and I haven't had any unusual activity since. But still keeping an eye on things.

-Fraud report filed
-All CCs are with Chase- they set up a verbal password, deleted the unrecognized phone number/address, issued a new card that was compromised, and had me change my online password
I just need to call my bank now. What happens after the 90 days? It's not like these people will just forget my SSN. I'm sure this could lead to big issues in the future too right?

I went through this last fall. Really the best way to go about this is to just pull your free yearly credit report and look through it. Since this has happened I have not been auto approved for anything. Every account I open I have to get a phone call to verify and they go over some security questions that I verbally set up with one of the credit agencies.

So I guess even if someone has your info, they don't always use it? This person changed my credit card address in early March. I didn't catch it until a couple weeks later. No accounts have been attempted to be open. Why change the address and not try to open accounts?

I'm not sure of the method to their madness. In my case, they hacked into my online account and made a payment on the card, paying off the entire balance with my checking account info saved online. They then attempted to use the card a couple of places, and that's when the card company caught it and contacted me. I'm still baffled how they got into my online account, and why they made a payment. There was plenty of available credit on it.
They also got into my other online account with another credit card. They applied for new cards with both accounts.
This was a few weeks ago and nothing new has developed. Seems like they hit for as much as they can, then move on to someone else. But I don't know.
