Probably should book now if you will need to do it. I think last time I looked it was $690 for over 18,000 mqm's... not too shabby.

I see a lot of opportunity for IAH to New Delhi (KLM) pretty frequently. Basically India in general for mileage runs seems to be pretty good if you're into that sort of thing.

You did in 2013, but they are trying to do visa on arrivals for certain countries and I think US is up to be added/if not already added.
As for the never leaving the airport. No idea.
ETA: Since the US hasn't been added to VOA program yet, I would think they wouldn't let you on the plane without one, not knowing you aren't entering the country. I remember my passport being checked before boarding for this purpose when I went last year. I guess this leaves it as "something to keep an eye on."
