Found the answers to all of my questions regarding this play in the rule book.
I also found this which is interesting and probably applies to the catching incident at SELU:
If the catcher rolling the ball is not considered a "throw," then the 2 bases from the time the throw was made rule does not apply.

The appeals made under this section must take place before the next pitch, play or attempted play or before the pitcher and all infielders have left fair territory on their way to the dugout, if it is an inning-ending or game-ending appeal.
I also found this which is interesting and probably applies to the catching incident at SELU:

For the purpose of this rule, the act of fielding the ball or a faked or feinted throw is not considered a play; a “play” must be a legitimate attempt by a fielder to retire a runner.

You got it but most people think it's a fair ball b/c it hit the pitching rubber. I will dig up the book title.

is this the book? otherwise I can't find any that match the description you gave.

Yeah I went ahead and ordered that one and another one on the topic that looked interesting from Amazon.
