You're probably sliding upper body forward a little bit in the downswing, which changes the direction of your path and low spot. Pick a spot on the back of the ball and watch it until the ball leaves

I agree, this is my miss as well and exactly what I do when it happens.

keep the handle low at impact - ensure that you rotate the handle around your body as you rotate
