I didn't encounter her until NG+. Went to the clinic and she warns you about entering, I was like 'frick you' and charged in, ready to mess her up with my Beast Claws. I quickly ran back down the steps.

It's a damage boost that lasts about a minute, until the beasthood meter disappears.

See i never encounters any "imposter" at iosefka's clinic. I guess I waited too late to go there? When I got the the clinic, I ran into a couple of those blue alien looking dudes, and when I found iosefka, she was on a table holding her stomach in pain. To fight the imposter, do I need to go before a certain point in the game?

Yes, you did.

One of those was Iosefka. The imposter turned her into it. That is why her voice changes through the door after you find the cathedral ward. The one that drops the Iosefka's blood vial is her.

You found the imposter, who was doing experiments to create another offspring for the old ones. That is why she is in pain, she is prego. When you kill her you get one of the cords.

If you send anybody to the clinic the Imposter turns them into those alien looking things. Iosefka also gets turned into one. (the one who drops the vial) I have no idea how you managed to not find the Imposter.

Maybe you attacking caused the Imposter to die or move on somehow. Not sure. The lore goes like this.

At the beginning of the game it is Iosefka at the door. She is nice to you when you talk to the door.

Then when you find the Cathedral Ward the Imposter takes over the closed door. It is a different voice actor and she is more of a bitch to you. She also tells you to bring people you find in the city to the clinic at this point.

Then when you make it to the forbidden woods and come from the roof, Iosefka will be a alien looking monster, and whoever else you sent to the clinic. The Imposter will be up the stairs down the hallway. She will threaten you. You can fight the Imposter or leave her alone.

Finally after you kill Rom, the Imposter will be prego on the table dying. You can kill her then.

I wonder if that is what happened. i just send the begger in the woods to the clinic so next time I play I will see what happens in there. I haven't faced rom yet.

Spoilerish:

There is a chance of "killing" her through the door and she'll disappear when you get access to the clinic. THIS will likely freak some out (like me) but will eventually sort of fix itself after some progression.

Go back after killing Rom and see if she's there and pregnant. If not then it's not the end of the world you'll just need to make sure you get the other three cords to get one of the three endings. That is if you want to get all three without going through the game three times.

If someone is looking for umbilical cords, you don't even have to bother with Iosefka. Get one at the abandoned workshop, one from Arianna in lower cath ward and one from wet nurse.

I've fallen in love with the threaded cane in my new play through. I can't believe I never tried it before now.

gotcha. so i did kill the imposter and got dat umbilical cord. i beat the game a couple weeks ago and am on NG+. it's pretty freaking tough again.

I'm at the witch boss, and kicked her arse the first time around in 1 try, but have died like 3 times on NG+. wish i could find a blood rock or whatever the frick it's called so I could get my axe to +10. is that only found in the dungeons?

to get the "true" ending, yea. I'm not sure if it's really the true ending, I just know you get to fight an extra boss at the end...

In responce to Jombie's most recent post, I found getting the umbilical cord from the abandoned workshop the most difficult and tedious... making that jump is a pain in the arse for me for some reason

It's the canon ending, basically.

Getting to the workshop can be a pain. These games just aren't tailored for platforming, but I'd recommend going there if you haven't and if you don't feel like doing the Iosefka sidequest.

Arianna is in the ally below cathedral ward, where you find the Black Church set. She'll move below the ladder leading to the cathedral after Micolash, you can then get the cord.

You really think that? It is really easy in my opinion.
