2 cheap Atari 800 joysticks from regular use
1 Xbox 360 controller when my then 2-year old son grabbed it when I wasn't looking and dropped it on our hard floor

only 1 out of rage playing a WWE game online. I swung the controller by the cord against the floor once or twice.

CoD has ruined many a gaming controller in my house.
Probably 8-10.
