Ice Water and Fruit

When did "ice water" become ice, water and a lemon? I find it odd that I have to specify not to put lemon in my drink at restaurants.
Does anyone have insight into when and why this became standard practice?
As a side note, Baton Rouge has the best tasting water of anywhere I have ever been.

I'm with you on this one. It is just another way for germs to get in your system. germs on knife/cutting board while cutting lemons, germs on fingers picking up lemon to put in glass.
