Few thoughts here:
1) Get routine annual physicals. If you care enough about your loved ones to get life insurance, get a damn physical so you don't let some possible underlying medical issue go untreated.
2) The insurance company requires you to have a Dr but did not require a paramed? Any term policy worth having (which is few) generally requires a paramed be done for underwriting. If it's a guaranteed issue which means they don't require underwriting (everyone is accepted), don't get it unless you have serious medical conditions and you cannot find coverage elsewhere.
3) Don't shop for this yourself. Find an experienced INDEPENDENT life insurance producer who represents many companies and can help you navigate this. There are tons of options, and while one company you know and have heard of may have a decent term policy that you think is great, I promise there's another company that you haven't heard of that has a better product for you.

There is no such thing as a guaranteed six figure term policy. Just saying. Guaranteed applies to small final expense policies or accidental death policies. Maybe an oddball group policy here and there. Also, all term life policies require underwriting, it's just a matter of how much. Fully underwritten (exam bloodwork) or Simplified (answer questions and scrip check). Simplified policies are appropriate for many, many consumers, such as this one. Healthy people would do far better with underwritten policies. The price difference is generally +25-40%.
