I've actually had three people fail the shooting test. All were shooting J-frame revolvers.

I always give people a second attempt, a few days later.

Two were women who missed shots at 15 feet. Both later passed the test.

One was a guy who missed two shots at 6 feet, so we ended the test at that point. He never came back -- sent me an email saying he had "changed his mind" about needing a permit.

I also tell my students that they will fail, and will not be given a second attempt, if they point their weapon at themselves, at me or at another person while at the range. Haven't had to fail anyone yet for this, but one rocket scientist (literally a NASA engineer) came within a few inches of failing.

The rocket scientist attempted to look down the muzzle to figure out why the slide didn't lock open. His hand got bruised when I slammed the pistol down onto the metal "shelf" on the firing lane. (I then explained to him that it didn't lock open because the pistol wasn't empty). I determined that he knew better (intellectually) than to look down the muzzle -- but scientific curiosity got the better of him.
