Electrical Types: Help Finding a Replacement Floor Outlet Cover

So, I'm an idiot (understatement and stating the obvious).

We replaced our floors recently and have two single receptacle floor outlets in our living room. I tossed the outlet covers when I pulled up the old floors thinking I'd find new ones to replace them. Now I'm having a very difficult time finding the correct size/manufacturer to replace the covers.

I've contacted Thomas&Betts/Steel City, Hubbell, and Walker/Wiremold and none of these recognize the outlet box as theirs.

So now I'm trying to figure out my next options. It's a concrete slab with hardwood on top. Can I somehow remove the old box, make the hole bigger, and rewire with a newer/larger box that I can source locally?

Any other options??

How old is the house ?

Is it PVC - Carlon ?

Raised house or slab?
Round or rectangular box/cover?

Single receptacle usually means a rectangular cover and also it's in a raised home.
If so, replace all of it if you're not sure.
Sorry, just read the rest, as others have said pics would be nice.

Ha! Same issue here. Covers for floor receptacles seem to be as individually unique as snowflakes.
