I just did it. There is only 1 "code" to be used.
The $40 is just referral credit which automatically applies to your account.
The $50 off is by applying code "SEMBBF16" on the last page. Make sure you replace any other code if there is one already there.
The Cashback is from the portal (Again no code).

Is there a way to see the $40 referral credit confirmed posted to your account before checking out? When I checkout it says $49 total with the $50 cyber monday promo applied.
